Required Immunization Records, Athletic Forms & Physicals
New students must provide an immunization record and physical to the Registrar before orientation day. Current students are strongly encouraged to contact their physician to be certain of immunization status prior to the beginning of each school year. ALL immunization records and physicals MUST be up-to-date by orientation.
LHSAA/Medical History Form
ALL NEW students and ALL ATHLETES are required to have a LHSAA/physical on file before the school year begins. New student Physicals are due by July 1. Athletes' physicals are due before the school year begins.
ALL students must provide an updated immunization record by July 1. Please make an appointment with your son’s physician to update his immunization record.
High School Athletes ONLY
If you are a High School Athlete in grades 8 - 12, for your convenience, physicals will be provided by Dr. Kevin Watson, HC’s team physician and Mrs. Myra DiGange, RN.
Student athletes may turn in all LHSAA paperwork at time of physical. LHSAA paperwork can be found on our website under “Athletics” the section titled "Athletic Forms - Master." (click here)
If your son is participating in physicals at Holy Cross, Please have these forms completed and ready to turn in to the Athletic Director on the day of the physical.
If your son does not participate in physicals at Holy Cross, the Athletic Director will be collecting LHSAA forms at orientation.

After School Care Policy
In order to provide a structured program and safe environment for students between the hours of 2:30 and 5:30 p.m., Holy Cross School has an after school care program. The program provides supervised time for homework and recreational activities. The program is mandatory for students who are on campus and not participating in Holy Cross supervised extracurricular activities. Teachers will plan their after school activities so that parents can make arrangements to ensure that their sons are picked up, ride the shuttle or attend after school care. A teacher will continue to supervise the pickup line until 3:00 p.m. each day at which time any students who have not been picked up will be escorted to the After School Care Program in the Cafeteria. The daily rate is $10. The program will be supervised each day by faculty member Mr. R. Charboneau.
- Students should report to the Cafeteria right after school
- Supervised homework/study time and snack—2:30‐3:30 p.m.
- Optional continued study time/outside recreational activities—3:30 ‐5:30 p.m.
* Pickups after 5:30: If a child is not picked up by 5:30, there will be an additional fee of $10 for that day due when the student is picked up.
Payment for the program:
- Payments are to be made via Quick Links on the website under Parent Login. Then select After Care Payment. Attendance and balance information is available at this site. Payments should be made in advance of attendance.
