Required Immunization Records, Athletic Forms & Physicals
|
Immunization Records
New students must provide an immunization record to the Registrar on or before orientation day. Current students will receive notification from the Registrar when immunizations are due, but are encouraged to contact their physician to be certain of immunization status. ALL immunization records MUST be up-to-date by orientation.
Required Athletic Forms
Parents/Guardians:
ALL NEW students in grades 5 - 12 and ALL high school ATHLETES in grades 9 - 12
are required to have a physical on file before the school year begins.
Athletes in grades 9-12 are required to have an LHSAA physical completed.
ALL students must provide an updated immunization record on orientation day.
Please make an appointment with your son’s physician to update his immunization record.
High School Athletes ONLY
If you are a High School Athlete in grades 9 - 12,
for your convenience, physicals will be provided by
Dr. Kevin Watson, HC’s team physician and Mrs. Myra DiGange, RN.
The cost is $20, cash only payment.
We are happy to announce that Dr. Kevin Watson will be providing LHSAA physicals to Holy Cross High School athletes on Tuesday July 5th from 1pm-4pm in the Athletic Training Room. Cost is $20 cash or check made out to “Holy Cross School.”
Student athletes may turn in all LHSAA paperwork at time of physical. LHSAA paperwork can be found on our website under “Athletics” the section titled “Athletic Forms.”
Additionally, student athletes in grades 9-12 are required
to have on file the following (4) documents for participation in the LHSAA:
- Assumption of Risk
- LHSAA Parental Permission
- LHSAA Medical History (the form used for the physical)
- LHSAA Substance Abuse
(These documents can be found on the HC website under "Athletics" by clicking on "Athletic Forms.") here
If your son is participating in physicals at Holy Cross, please have these forms completed and ready to turn in to the receptionist in the administration building on the day of the physical.
If your son does not participate in physicals at Holy Cross, the Athletic Director will be collecting LHSAA forms at orientation.

What are Tiger Prep Days?
|
Tiger Prep Days are a full-day orientation designed to introduce incoming students and their parents to Holy Cross School. The day is aimed at preparing families for the Holy Cross experience, which engages the heart, mind, body and soul. The goal is facilitate a smooth transition for incoming students and parents by developing the skills necessary for first year success.
Tiger Prep Days include events for both incoming Parents and incoming Students. Students will participate in Class Retreats lead by Campus Ministry Students, Eucharistic Liturgy, Lunch in the Huddle and Laptop Orientation and Distribution. Parents will participate with their sons in Eucharistic Liturgy and Lunch in the Huddle and then attend a Counseling and Guidance Information Meeting and Internet Safety Training. Tiger Prep Days strive to give a comprehensive overview of Holy Cross School while welcoming our new families with opportunities to socialize and participate in training sessions.
Students Must wear uniforms.

Dear Holy Cross Parents:
In order to provide a structured program for students between the hours of 2:30 and 5:30 p.m., Holy Cross School has an afterschool care program. The program provides supervised time for homework and recreational activities. The program is mandatory for students who are on campus and not participating in Holy Cross supervised extracurricular activities. Teachers will plan their after school activities so that parents can make arrangements to ensure that their sons are picked up, ride the shuttle or participate in after school care. A teacher will continue to supervise the pickup line until 3:00 p.m. each day at which time any students who have not been picked up will be escorted to the after school program. If prepayments are not made, the daily rate will be $10 per student payable upon pick up that day. The program will be supervised each day by Holy Cross facutly member Ray Charboneau. An additional Holy Cross teacher will work with the program each day.
Daily schedule:
- Supervised homework/study time and snack—2:30-4:00 p.m.
- Optional continued study time/outside recreational activities—4:00 -5:30 p.m.
- The program will begin at 2:10 p.m. on early dismissal days—Tuesdays.
- The daily rate is $8. per day until 5:30 p.m. when paid in advance.
- Parents can choose which days over a period of time that they would like for their sons to attend as long as prepayments are made in $40 increments.
*Pickups after 5:30: If a child is not picked up by 5:30, there will be an additional fee of $10 for that day due when the student is picked up.
*No after school program on 11:00 dismissal days.
Payment for the program :
- Payment should be made in increments of $40 in advance.
- Checks should be in a sealed envelope with the following information on the envelope: student’s name, the amount enclosed and After School Program.
- Payments may be mailed to or dropped off at Holy Cross to the attention of the After School Program.
- Checks should be made to Holy Cross School.
